• If this is your first time seeing Dr. Mallard, please select the New Client appointment option.

• If you’ve previously seen Dr. Mallard either here or at the Tahoma Clinic, please select the Follow-Up and Ongoing Support appointment option.

Time Zone Notice

All appointments are scheduled in Pacific Standard Time (PST) by default. To update your settings so appointment times are displayed in your time zone, please click here for instructions to change this on your account: Choosing Your Time Zone

Cancellation and Rescheduling Policy

Please double-check your time zone to avoid late cancellations or no-show fees. We require at least 24 hours’ notice for cancellations or changes. - Less than 24 hours’ notice: 50% of the appointment cost will be charged. - Missed appointments with no notice: The full cost of the appointment will be charged.
